Certification of Identity
The Certificate of Identity is issued to stateless people who are Permanent Residents of Malaysia for the purpose of travelling abroad.

Validity
• Five (5) years from the date of issue.
Application
• Form-IM.45
• Two (2) passport size photographs of applicant (3.5 cm x
5.0 cm)
• Previous / Old Malaysia Certificate of Identity (for
renewal)
Categories and Documents Required
• Identification Card and Entry Permit for applicant aged 18 years and above
• Identification Card, Marriage Certificate and Entry Permit for applicant aged
below 18 years and marriage
• Identification Card (12 years and above), Entry Permit, Birth Certificate and
Parent's Identification Card / Entry Permit / Passport for applicant aged below
18 years
• Identification Card (12 years and above), Entry Permit, Birth Certificate,
Parents' Death Certificate and Statutory Declaration of Guardianship for
applicant aged below 18 years but parents deceased
• Identification Card (12 years and above), Entry Permit, Birth Certificate,
Either Parent's Identity Card / Entry Permit / Passport and Court's Order for
Child Custody for applicant aged below 18 years but parents are divorced and
cannot be contacted
• Identification Card (12 years and above), Entry Permit, Birth Certificate,
Identification Card of Legal Guardian and Certificate of Adoption for applicant
which is adopted child aged below 18 years
