Lost and Damaged Malaysian Passport Replacement

Application for Malaysian Passport replacement for lost or damaged case can be submitted at any Immigration Offices in Malaysia or Malaysian Representative Offices abroad.

The documents required during application for replacement are as following:

1. Police report for a case of lost and stolen
2. Identification Card (MyKad / MyKid)
3. Birth Certificate and Parent's Identity Card (for applicant aged below 18 years old)
4. Questionnaire Form IM42c
5. Copy of Lost Passport (if any)
6. Marriage or Divorced Certificate (for applicant aged below 18 years old if Marriage or Divorced)
7. Confirmation letter of employment for whose working abroad (if applicable)
8. Offer Letter if study abroad (if applicable)
9. Emergency Certificate with a report from Entry Point Immigration Office (if applicable)
10. Verification of Status (for a Permanent Resident of third country)
11. Report of Fire & Rescue Department if the lost is caused by fire
12. Letter from Municipal Council if lost is caused by flood or other natural disaster

Approval for lost / damaged Malaysia Passport replacement is subject to the decision of Director General of Immigration and the result will be notify within 5 working days from the date of application submitted.

Lost or damaged case abroad should be report to nearest Malaysian Representative Office for replacement.
