This document is issued to Malaysian Citizens residing for six (6) months and above in the Administrative District of Pensiangan, Tenom, Sipitang and Tawau in the State of Sabah and Lundu, Bau, Kuching, Serian, Simanggang and Lubok Antu in the state of Sarawak. The validity of this document is one (1) year from the date of issue and valid for multiple journeys. Permission will be granted to stay in Indonesia 30 days for each entry.


Condition of Application

  Form (Imm.42-pin 1/97)
  Two (2) photographs of the applicant measuring 3.5 cm x 5.0 cm
  Children below 12 years of age cannot be issued with a separate document

12 years and above

  Two (2) photographs of the application measuring 3.5 cm x 5.0 cm
  Identification card (Mykad)

Below 12 years

  Two (2) photographs of the application measuring 3.5 cm x 5.0 cm
  Birth Certificate
